Organization Grants


Important Definitions

A sponsor is someone who is not formally affiliated with the organization receiving the grant. They are sponsoring an organization to receive the grant. If you are a sponsor, leave Section 2 of this form blank.

A representative is someone who is formally affiliated with the organization receiving the grant. They are filling out this form on behalf of an organization that they represent. If you are a representative of the organization receiving the grant, leave Section 1 of this form blank.


Section 1: Sponsor Information (if applicable)

If you are sponsoring a nonprofit or organization to receive this grant, please fill out the following information about yourself. Leave this section blank if the grant you are requesting is for a nonprofit or organization that you are affiliated with.

Please describe your relationship with the nonprofit or organization.


Section 2: Representative Information (if applicable)

If you are requesting that this grant be awarded to a nonprofit or organization that you are formally affiliated with, please input your information in the section that follows. Leave this section blank if you are sponsoring a grant for a nonprofit or organization that you are not affiliated with.

Please explain in depth how much money you request be awarded in this grant and what the grant will be used for if awarded.
